Fill out, print and bring or email 2 forms: medical history form and contract (found under "FORMS" tab).

Park in front U shaped driveway (house is on a corner lot) which leads to FRONT DOOR.

Location is 6730 N Palmerston Drive, Mentor, Ohio

SKYPE and phone visits are available, if necessary.
